Family Information Sheets are a perfect way to keep track of your family’s health insurance information. They are such a time saver when you get that dreaded health related bill that for some reason doesn’t have the health insurance information. Just pull out your page and fill in the information.

They are a great addition to your Home Management Binder and/or Emergency Binder/ Folder.

These sheets have:

  • Space for up to 4 family members
  • Each family member space has:
    • Name
    • Insurance Company
    • Date of Birth
    • Company Address
    • Company Phone Number
    • Group Number
    • Claim Number
